  • Observation: Observe the potential for a safety issue or problem to occur. For example, an uneven or slippery surface that could result in a trip or fall.
  • Near miss: Events that could have led to bodily harm or property loss but did no. For example, a worker trips over an uneven surface, but it does not result in injury.
  • Incident: An occurrence, condition, or situation arising in injury, illness, or fatality. For example, a worker slips on a slippery surface and it results in an injury.
